Role of Routine Ultrasonography in Preoperative Evaluation of Thyroid Nodules


Abstract
A high-frequency, high-resolution ultrasonography has been performed routinely in patients with thyroid nodule as a preoperative assessment of the thyroid in Dept. of Surgery, Pusan National University Hosp, since Mar, 1993. In this study,
findings
of
the ultrasonography examined in the first 100 consecutive patients were analyzed to evaluate its potential role in conducting operative procedure. They had been screened largely by fine-needle aspiration cytology in patients with low-uptake
solitary
nodule.
1) The primary nodule was benign in 60 cases (16.0%) and malignant in 40 cases (40.0%) on final in each group respectively. The mean age was 37.7 and 40.3 and male-to-female ratio was 1 : 76 and 1 : 5.7 in each group respectively. The operative
procedure was lobectomy plus isthmusectomy in 60 cases (60.0%) and total thyroidectomy in 31 cases (31.0%). Node dissection was performed in 6 cases (6.0%).
2) On ultrasonography. mutinodularity (presence of additional nodule or lesions larger than 3 mm in size) was found in 43 cases (43.0%), and was bilateral in 32 cases (32.0%). There was a remarkable contrast in frequency of malignant nodule
between
patients with multinodularity and those with single nodule-27.9% vs. 50.9%. The size of primary nodule was less than 2.0 cm in 15 cases (15.0%), and was between 2.0 cm and 4.0 cm in 66 cases (66.0%). Lateral node enlargement (larger than 1.0 cm
in
size). Either ipsilateral or bilateral. was found in 10 cases (10.0%) including 5 cases with clinically palpable nodes.
The high-resolution ultrasonography was very useful in assessing thyroid preoperatively, and was also very helpful in planning and conducting details of surgical procedure for benign and malignant thyroid nodules. It is strongly recommended as a
routine
preoperative procedure for the patients with thyroid nodule.
